This will be the finished result:

Open a new file, 800 x 800, 72px resolution, white background
Create a new layer
Select the Brush Tool and Brush Hearts set
Select brush number 9, set size to 60px
Set foreground colour to red #E51F0B
Create a background of hearts as shown below (don’t be too careful with the placement of each heart):
Select the Eraser Tool and Heart Brushes by Tatsuya Saverem
Select brush number 7, set the size at 520px
Click about 6 times in the same place to erase the heart shape
Create a new layer above the background layer
Select the Brush Tool and brush number 7 as above, size 520px
Foreground colour should still be red #E51F0B
Place the heart into the gap you erased and click once
Make the many-hearts layer active and using the Eraser Tool or your preferred method, erase the hearts from the outside of the bigger heart shape
Double click on the layer in the layers palette to bring up the blending options and apply the following Bevel & Emboss settings:
Double click on the heart outline layer in the layers palette to bring up the blending options and apply the following Drop Shadow settings:
Open the Cupid image
Select>All
Edit>Copy (Ctrl + C)
Make your project active
Edit>Paste (Ctrl + V)
Double click on the layer in the layers palette to bring up the blending options and apply the following Drop Shadow settings:
Select the Text Tool and Hole-Hearted font, size 120pt, foreground colour black
Set alignment to right-align
Type ‘Stupid (enter key) Cupid’
Right click on the layer in the layers palette and click Rasterize Type
Set foreground colour to red #E51F0B
*Select the Magic Wand Tool
Click in the heart shape of the P
Select>Modify>Expand – set to 1px, click OK
Create a new layer
Select the Fill Bucket and click inside the selection**
Repeat from * to ** for the D of Stupid and P and D of Cupid
Make the heart layer of one of the Ps active
Right click on the layer in the layers palette and click Duplicate Layer
Select the Move Tool and move the heart to the heart shape in the C
Using the Magic Wand Tool, select the heart above one of the Is
Create a new layer
Fill the selection with red as before
Repeat for the heart above the other I
Double click on any of the text heart layers to bring up the blending options and apply the following Bevel & Emboss settings:
Copy and paste the layer style to all text heart layers
Double click on one of the hearts above the Is to bring up the blending options and apply the following Stroke settings:
Apply the same stroke settings to the other heart above the I
Turn off the visibility of the layers containing the hearts above the Is
Make the text layer active
Select the Eraser Tool and erase the hearts above the Is
Restore visibility to all layers
Double click on the text layer in the layers palette to bring up the blending options and apply the following Gradient Overlay, Bevel & Emboss and Stroke settings:
Create a new layer above the background layer
Select the Gradient Fill Tool
Set the gradient as shown below:
Set to Radial Gradient, opacity 65%
Drag the cursor from the middle of the image to any corner
In the layers palette drag the cupid layer to the top of the palette
